COVID-19 Vaccine

  1. Rendr now offers the updated bivalent COVID-19 boosters to individuals 12 years and older at least two months after their most recent vaccine dose.
  2. Rendr offers COVID vaccines to individuals 6 months and older; booster shots to individuals 5 and older; additional primary series dose to individuals who are immunocompromised.
  3. Rendr offers COVID vaccines to children 6 months and older. Vaccines for children ages 6 months to 5 are ONLY available at Rendr pediatric practices, as listed in Vaccination Locations.
    • The Pfizer-BioNTech vaccine:
      • Children 6 months to 4 are eligible for the three-dose Pfizer vaccine. The first and second doses are separated by three weeks, and the second and third doses are separated by eight weeks.
      • Children 5 and older are eligible for the two-dose Pfizer vaccine, given three weeks apart.
    • The Moderna COVID-19 vaccine:
      • Children 6 months to 5 are eligible for the two-dose Moderna vaccine, given one month apart.
  1. A booster is recommended for everyone 5 and older. Children 5 and older should receive a booster dose of the Pfizer vaccine at least 5 months after completing their initial Pfizer vaccine series. Children who have received Moderna vaccine as their primary series are not eligible for a booster dose at this time.
  1. Rendr offers an additional primary series dose of the COVID vaccine to individuals who meet following three conditions:
    • Aged 6 months and older if you receive Moderna; or aged 5 and older if you receive Pfizer;
    • Received two doses of the Pfizer or Moderna vaccine at least 28 days ago;
    • Considered moderately to severely immunocompromised.

For those who received one-dose J&J vaccine and are immunocompromised, an additional dose of mRNA COVID vaccine at least 28 days later is recommended.

What is the process?

Step 1

Call Rendr vaccine locations to make an appointment (9am – 5pm Monday – Friday). Vaccination eligibility (proof of age, etc.) will be verified.

Step 2

After the appointment is confirmed, complete and submit Vaccine Form online before administration.

Step 3

Arrive at the chosen location on scheduled time with all personal identification and consent forms.

Step 4

Answer medical screen questions from healthcare providers.

Step 5

Get COVID-19 vaccine administered.

Step 6

Wait for 15-30 minutes under observation to ensure no severe reactions.

Step 7

Checkout and schedule appointment for the next dose.


Required documents for visit

After verifying your eligibility with the clinic and confirming your appointment, please prepare the following categories of documentation for your visit.

1. Proof of Eligibility

Please electronically fill in this government required form. We strongly encourage you to complete and submit the form prior to your appointment. When you arrive at the vaccination sites, show your submission confirmation to the staff.

Prevent COVID-19

Protect yourself and others around you by knowing the facts and taking appropriate precautions. Follow advice provided by your local health authority.

To prevent the spread of COVID-19:

  • Clean your hands often. Use soap and water, or an alcohol-based hand rub.
  • Maintain a safe distance from anyone who is coughing or sneezing.
  • Wear a mask when physical distancing is not possible.
  • Do not touch your eyes, nose or mouth.
  • Cover your nose and mouth with your bent elbow or a tissue when you cough or sneeze.
  • Stay home if you feel unwell.
  • If you have a fever, cough and difficulty breathing, seek medical attention.

Calling in advance allows your healthcare provider to quickly direct you to the right health facility. This protects you, and prevents the spread of viruses and other infections.

Treatments

  • Call your health care provider or COVID-19 hotline to find out where and when to get a test.
  • Cooperate with contact-tracing procedures to stop the spread of the virus.
  • If testing is not available, stay home and away from others for 14 days.
  • While you are in quarantine, do not go to work, to school or to public places. Ask someone to bring you supplies.
  • Keep at least a 1-metre distance from others, even from your family members.
  • Wear a medical mask to protect others, including if/when you need to seek medical care.
  • Clean your hands frequently.
  • Stay in a separate room from other family members, and if not possible, wear a medical mask.
  • Keep the room well-ventilated.
  • If you share a room, place beds at least 1 metre apart.
  • Monitor yourself for any symptoms for 14 days.
  • Call your health care provider immediately if you have any of these danger signs: difficulty breathing, loss of speech or mobility, confusion or chest pain.
  • Stay positive by keeping in touch with loved ones by phone or online, and by exercising at home.
